Welcome to HEAL Wellness LLC, doing business as HEAL Wellness + Longevity ("HEAL," "we," "our," or "us"). By accessing our website, booking services, purchasing memberships or credit packs, or visiting our facility, you agree to these Terms of Service ("Terms"). Please read them carefully. If you do not agree with any part of these Terms, you may not use our website or services.
These Terms work alongside our Privacy Policy, Membership Service Description, Cancellation Policy, Liability Waiver, and (where applicable) the BIPA Consent for STYKU. Where there is any conflict, the more specific document controls for that subject.
1. Use of Our Website & Services
By using our website or booking any HEAL service, you represent that:
- You are at least 18 years of age, or
- You are 13–17 with verified parental/guardian consent and supervision as required.
- No one under 13 may use HEAL facilities.
- You will not use our website or services for unlawful or prohibited purposes.
HEAL reserves the right to refuse service or remove any guest whose behavior compromises the safety, comfort, or well-being of other guests, contractors, or staff.
HEAL further reserves the right to deny any service to any guest under 18 at any time, for any reason, at HEAL's sole discretion. Parental or guardian consent does not entitle a minor to service.
Shared Contrast Therapy suite — both-under-21 prohibition. A private Contrast Therapy suite may be shared by two guests only if at least one of them is 21 or older. If both guests are under 21, they may not share a CT suite together. This applies across the board, regardless of relationship, and includes siblings.
2. Wellness Services Only — Not Medical Care
HEAL provides wellness, recovery, and biohacking services to support general well-being. HEAL's services are not a substitute for medical diagnosis, treatment, or emergency care.
By using any HEAL service, you acknowledge that:
- You are responsible for your own health and safety.
- You will seek qualified medical advice for any health concerns.
- You will disclose relevant health conditions to staff before each session.
- Certain services — including infrared sauna, cold plunge, red light therapy, PEMF, Tesla Light Therapy, hyperbaric oxygen, lymphatic therapy, and others — may not be appropriate for individuals with specific medical conditions. Always consult your physician before beginning any wellness program.
2a. Medical Services Are Provided by Asher Med LLC
Any medical services available on site — including peptides, hormone optimization, GLP-1 weight-management programs, and telehealth consultations — are provided by Asher Med LLC, an independent, separately licensed medical entity. Asher Med is solely responsible for its clinical services, recordkeeping, patient relationships, and applicable regulatory compliance (including HIPAA where applicable). HEAL does not provide medical care.
2b. Acupuncture Is Provided by Pearl Lasky, L.Ac.
Acupuncture services on site are provided by Pearl Lasky, L.Ac., an independent licensed acupuncturist contracted with HEAL. Pearl Lasky is solely responsible for the clinical acupuncture services she provides.
3. Booking, Credits, Memberships & Billing
3a. Appointments
Appointments may be booked online through Momence, by phone, or in person. Availability is not guaranteed.
3b. Payment
Payment is due at the time of booking unless otherwise stated. We accept major credit and debit cards through our PCI-compliant payment processor. HEAL does not store full card numbers.
3c. Credit System
Most HEAL services are priced in credits. Credits may be used from:
- An active monthly credit membership (Core, Thrive, or, for legacy members, Elite), or
- A pre-paid credit pack purchased from HEAL.
Credits are deducted at the time of booking. Credit prices are posted in Momence and may be updated from time to time.
3d. Credit Packs — 6-Month Expiration
All credit packs purchased on or after June 14, 2026 expire six (6) months from the date of purchase. Unused credits remaining at expiration are forfeited and are non-refundable. Specific pack details, including the Member's 10 pack and Founders packs, are described in the posted pack catalog.
3e. Memberships
Memberships are month-to-month and renew automatically each billing cycle unless canceled in accordance with the Cancellation Policy. Each membership delivers a recurring credit allowance per the Membership Service Description.
- Unused membership credits at the end of a billing cycle are managed per the Membership Service Description and do not accumulate indefinitely.
- No refunds will be issued for unused credits or sessions within an active billing cycle.
- It is your responsibility to manage your membership through your Momence account or by contacting us.
3f. Cancellation of Bookings
Cancellation, late-cancel, and no-show rules are governed by HEAL's published Cancellation Policy. Late cancellations and missed appointments may result in a fee or loss of the credits used to book the session.
3g. Non-Transferable
Memberships, credits, and credit packs are personal to the purchaser. They are non-transferable and may not be shared, gifted, or assigned to another person.
4. Gift Cards & Service Packages
Gift cards and service packages:
- Are non-refundable.
- Are non-transferable unless explicitly stated.
- Must be used within the validity period stated at purchase (credit packs sold on or after June 14, 2026 expire 6 months from purchase).
- Cannot be redeemed for cash.
5. Conduct & Facility Rules
To maintain a safe and respectful environment:
- Follow all posted rules and staff instructions.
- Use equipment and facilities responsibly.
- Refrain from disruptive, unsafe, or inappropriate behavior.
- Shower before using the cold plunge. Failure to shower will result in an additional cleaning fee.
- Nudity is permitted only inside private Contrast Therapy suites.
- No animals other than ADA-defined service animals.
- HEAL may remove or ban any individual who violates safety or conduct guidelines.
6. Safety & Assumption of Risk
By using HEAL's services, you acknowledge and accept the inherent risks of wellness therapies, including but not limited to:
- Heat and cold exposure
- Physical exertion
- Light, sound, vibration, oxygen-pressure, and electromagnetic exposures
You agree to complete and sign HEAL's Liability Waiver before using HEAL services and to comply with the safety information posted on our Health & Safety page. You voluntarily assume all risks and release HEAL from liability for injuries, adverse reactions, or health issues arising from your use of our services, except where prohibited by law. Nothing in these Terms limits liability that cannot be limited by law.
7. Biometric Information (STYKU)
Use of HEAL's STYKU 3D Body Scan is voluntary. Because the scan collects biometric information under the Illinois Biometric Information Privacy Act (740 ILCS 14, "BIPA"), HEAL will provide you with a written BIPA disclosure and obtain your signed, written consent before performing your first scan. You may decline or revoke consent at any time. See our Privacy Policy and BIPA Policy for full details, including retention and destruction.
8. Product Purchases
HEAL may sell wellness products, supplements, bottled water, branded merchandise, and devices. Product availability and pricing may change at any time. All product sales are final unless defective in accordance with the manufacturer's policy.
9. Intellectual Property
All website content — including text, images, graphics, logos, brand marks, and designs — is the property of HEAL or our licensors and may not be copied, distributed, or used without written consent. "Plunge," "HigherDOSE," "FilterBaby," "STYKU," and other brand names referenced on our site are the property of their respective owners and are referenced only to identify the equipment we use.
10. Third-Party Links & Services
Our website may contain links to third-party websites or services. HEAL is not responsible for:
- The content of external sites
- The policies or practices of third-party services
- Any damages or losses arising from your use of external sites
Use third-party resources at your own discretion.
11. SMS Program Description
By opting in, you agree to receive SMS/MMS messages from HEAL Wellness LLC (d/b/a HEAL Wellness + Longevity) for purposes including promotional offers, appointment reminders and confirmations, billing notices, and customer support. Message frequency may vary based on your activity.

- Opt-in Consent: By submitting your phone number, you authorize HEAL to send you text messages and notifications. Message and data rates apply.
- You explicitly consent to receive automated marketing and transactional messages.
- We will not share mobile information with third parties/affiliates for marketing or promotional purposes. Subprocessors (such as our SMS provider, support tools, and aggregators) may access the data solely to deliver the SMS service.
- Originator opt-in data and consent information will not be shared with any third parties, excluding aggregators and providers of the text message services.
- To unsubscribe at any time, reply STOP to any message. A confirmation will be sent and no further messages will be delivered. Reply HELP for assistance or email info@heal4wellness.com.
Message & Data Rates Disclosure: Message and data rates may apply for any messages sent to or from you. You will typically receive 2–5 messages per month. Contact your wireless provider with questions about your plan.
Carrier Liability Disclaimer: HEAL and its carriers are not liable for delayed, failed, or misdelivered messages due to network or device issues.
12. Privacy
Your use of our website and services is also governed by our Privacy Policy, which explains how we collect, use, and protect your information, including biometric information collected through STYKU under BIPA. By using HEAL, you agree to both these Terms of Service and the Privacy Policy.
13. Modifications to Services or Terms
HEAL may update or modify services, pricing, the credit system, packs, memberships, or these Terms at any time. Changes will be posted on this page with an updated effective date. Continued use of our website or services after the effective date constitutes acceptance of the updated Terms.
14. Governing Law & Dispute Resolution
These Terms are governed by the laws of the State of Illinois, U.S.A., without regard to its conflict-of-law principles. Any dispute arising out of or relating to these Terms or your use of HEAL services will be resolved in the state or federal courts located in Cook County, Illinois, unless the parties agree otherwise in writing.
